Dr Peter Anthony Haerstch - unsatisfactory professional conduct not proved
The Health Care Complaints Commission prosecuted Dr Peter Haertsch, a plastic and reconstructive surgeon, before a Medical Professional Standards Committee at an inquiry held on 20, 21 and 22 October 2014.

John Michael Patrick Ashford - unsatisfactory professional conduct, professional misconduct, impairment and not a suitable person
The Health Care Complaints Commission prosecuted John Michael Patrick Ashford, a former registered nurse, before the New South Wales Civil & Administrative Tribunal. The prosecution concerned Mr Ashford’s conduct during his employment at St George Hospital in February 2013 when he took nine Oxycontin tablets for his own use. The complaint alleged that Mr Ashford had provided false and misleading information to hospital staff and the NSW Nursing and Midwifery Council in response to this conduct. It was further alleged that Mr Ashford misled the Council in relation to misappropriating Fentanyl whilst employed at Westmead Hospital in July 2013.

The Health Care Complaints Commission prosecuted Halina Bialowas, a Registered Nurse, before the NSW Civil and Administrative Tribunal.

Mr Andrew Ferguson - finding of impairment by the NSW Civil and Administrative Tribunal and reprimanded
The Health Care Complaints Commission prosecuted Andrew Ferguson, an enrolled nurse, before the NSW Civil and Administrative Tribunal. The prosecution arose from Mr Ferguson’s conduct whilst working at the Hay Hospital where he presented for work intoxicated in December 2012.

Dr Farukh Tehseen - unsatisfactory professional conduct, reprimanded and conditions imposed
The Health Care Complaints Commission prosecuted Dr Farukh Tehseen, a general practitioner, before a Medical Professional Standards Committee. In April 2012 Dr Tehseen was a Career Medical Officer at Wollongong Hospital and treated a patient who presented to the Emergency Department experiencing a possible psychotic episode.

Mr Gareth Michael Dawes - unsatisfactory professional conduct, professional misconduct, impairment and lack of competency
The Health Care Complaints Commission prosecuted Mr Gareth Michael Dawes, a psychologist, before the NSW Civil & Administrative Tribunal. The Commission’s prosecution concerned Mr Dawes’ conduct in relation to a female client. The Commission alleged Mr Dawes did not maintain appropriate professional boundaries with the client including exchanging in excess of 3,000 emails, a large proportion of which included sexual content, and engaged in inappropriate sexual contact and behavior with the client. The Commission also complained that Mr Dawes suffered from an impairment in relation to his sexual preoccupations. The Commission also prosecuted a complaint that Mr Dawes lacked competency to practice psychology because he does not have sufficient mental capacity to do so. At the time of the inquiry, Mr Dawes was no longer registered as a psychologist.

Ms XC (formerly Dr XC) - unsatisfactory professional conduct, professional misconduct, impairment and lack of competency
The Health Care Complaints Commission prosecuted Ms XC, a former medical practitioner, before the New South Wales Civil & Administrative Tribunal. The prosecution concerned Ms XC’s addiction to Stillnox and other prescription drugs between 2007 and 2012. The complaint also alleged that Ms XC had breached a number of conditions on her registration and that she had improperly used her supervisor’s prescription pad to self prescribe. It was also alleged that Ms XC had misled the Medical Council of NSW.

Dr Jassim Daood – unsatisfactory professional conduct found, reprimanded, and conditions imposed
In September 2012 the NSW Coroner forwarded findings of an Inquest to the Health Care Complaints Commission for its consideration whether to bring proceedings against Dr Jassim Daood, a general practitioner. The Commission investigated the complaint and prosecuted Dr Daood before a Medical Professional Standards Committee.
